500			_aThis study identified the model that represents the fertility rate status of women in Bayombong, Nueva Vizcaya, consequently describing the profile of the respondents. concentrations and the discriminating factors of high and low fertility rate. Purposive sampling was used in selecting respondents from the 25 barangays of Bayombong. It used descriptive correlation with exploratory design. Particularly, it used frequency count to describe the profile of the respondents. Furthermore, it used Spatial Autocorrelation (Moran's 1) and Heat Map in describing the concentrations of the data and Linear Discriminant Analysis in finding the determinants and model of fertility rate. The results revealed that concentrations of high and low fertility rate are in the urban center of the municipality and the discriminating variables include the educational attainment of the respondents, their choice of facility where they get their health services, their age, their experience of child death, their choice of school for their children, their use of contraceptives and the method of contraceptive that they use. Finally, the model revealed specific characteristics of women belonging to a certain fertility status.
