N1  - The study aimed to compile test questions for first quarter examinations of all teachers teaching Mathematics 2 in the District of Villaverde, Nueva Vizcaya representing all the different learning competencies and classified as LOTS and HOTS, to propose two parallel forms of first quarter examination tests which include the tests characteristics, and to propose steps in drafting District-wide Quarter Examination test questions. Other objective of the study is to determine the characteristics of the teacher-made quarterly examination particularly the First Quarter Examination in Elementary Mathematics for Grade two of Villlaverde District, Villaverde Nueva Vizcaya. The study was conducted to grade 2 classes of all public schools in the District of Villaverde during the school year 2020-2021 using a descriptive type of research. Twelve teacher -respondents teaching Mathematics 2, and 201 randomly selected pupil-respondents from the 9 schools in the district were directly involved in the study. Formula and indices in determining the content validity, difficulty index, discrimination index, reliability coefficients for test stability, parallel forms, and internal consistency were used. The study was able to come up with a Test Item Pool for first quarter examination, two parallel tests Set A and Set B which were valid and reliable
